The DAISY and PETALS Award Nomination

About the DAISY Award

Has a nurse at Reedsburg Area Medical Center/Reedsburg Area Senior Life Center made a difference that you'll never forget? Say thank you by nominating them for the DAISY (Diseases Attacking the Immune System) Award. This international recognition program honors and celebrates the skillful, compassionate care nurses provide every day.

The DAISY Foundation was established by the family of J. Patrick Barnes after he died from an auto-immune disease ITP in 1999. During his hospital stay, his family deeply appreciated the care and compassion that Patrick received. When he died, his family felt compelled to say "thank you" to nurses in a very public way. 

Each DAISY Award Honoree will be recognized at a public ceremony in her/his unit and will receive the following: a beautiful certificate, a DAISY Award pin, and a hand-carved stone sculpture entitled A Healer's Touch. Additionally, everyone in the unit will celebrate with cinnamon rolls – a favorite of Patrick's during his illness. The Barnes Family asks that whenever and wherever nurses smell that wonderful cinnamon aroma, they stop for a moment and think about how special they are.

About the PETALS Award

The PETALS Award was developed to recognize the many deserving non-nursing team members who have the skills and dedication to provide excellent customer service and/or patient care outside the nursing profession.

Whether you are a patient, family member, visitor, clinician or staff member, if you know of a team member at RAMC/RASLC who is an outstanding role model of service, please use the form below to nominate them for the PETALS Award.

P – Passion/compassion for the patient. Does the nomination show the staff member is compassionate about the situation?

E – Empathy in the situation. Does the staff member show empathy toward the individual or situation in the nomination?

T – Trust and teamwork of families, patients and peers. Is there a sense of trust in the staff member that is conveyed in the nomination?

A – Admirable attributes possessed. Are there some attributes that others would admire in this staff member conveyed in the nomination?

L – Love for the patient and healthcare profession.

S – Selflessness. Is there any selflessness on the part of the staff member?
